Information - Returners
We will continue to use Healthy Roster to collect and store your health and medical information. Starting in June, you will receive a reminder email from Healthy Roster notifying you that you must take action with your online Student Medical Record (SMR). This email will list any incomplete requirement that is due or any existing requirement that will expire and needs updating. You will receive instructions at each step of the process upon login. Simply login to your SMR and you will find:
- Information Saved From Last Year Will Be Displayed
- Vital Health Record: You will need to complete a questionnaire regarding any changes to your health since the completion of your initial Vital Health Record. Fill out each question appropriately, and give all details and documentation for any changes.
- Information May Only Require A Digital Signature To Complete
- Digital Authorization of Forms: All forms are e-signatures. They are accessed through your Healthy Roster account, and will be labeled as "Expired", "Expiring", or "Missing". Carefully read each document, fill out all required spaces, and sign with your first and last name.
- Information That Must Be Reviewed And Updated Each Year
- Medical Forms/Annual Doctors Forms: You must update any pertinent medical information and then submit these forms or letters back to Healthy Roster. These forms may include medications, allergy action plans, information regarding recent injuries, and more. They should be submitted to the "Incoming Medical Documents' document. A template is available to use as a checklist for all information needed.
- Insurance Card: A resubmission of your medical insurance card will be required, even if there have been no changes. You will need to upload a copy of both the front and back of your insurance card to fulfill the requirement.
When you begin receiving emails from Healthy Roster, log on and complete the information required.
